West Hertfordshire Teaching Hospitals NHS Trust is seeking an experienced Rotational Advanced Pharmacist on a fixed-term basis to take a senior clinical role across Medicine, Care of Older People and the Acute Admissions Unit, providing expert pharmaceutical advice and supporting the safe and effective use of medicines.

You will work with multidisciplinary teams, contribute to service development and medicines optimisation, and support the development of junior pharmacy colleagues.

Qualificações

• Senior clinical role across Medicine, Care of Older People and the Acute Admissions Unit.
• Provide expert pharmaceutical advice and ensuring safe and effective use of medicines.
• Assist in developing junior pharmacy colleagues.

Responsabilidades

• Assist the lead pharmacists in the delivery, development and evaluation of a high-quality clinical pharmacy service for Older People, Medicine and Acute Admissions Unit.
• Provide expert advice on pharmaceutical matters, undertaking risk management and ensuring compliance with medicines legislation.
• Participate in clinical audit and practice research.
• Undertake training of pharmacy staff and other members of the multi-disciplinary team.
• Actively deprescribe as a fully independent pharmacist prescriber and provide training in deprescribing tools to support reduction of polypharmacy in frail patients.
