We are looking for a Senior Registered Nurse who can think on their feet and respond to rapidly changing situations. In this key role, you will support the development of a skilled, effective workforce and coordinate and manage the caseload of patients receiving home nursing and support at home services. As a proactive member of the Home Nursing Services team, you will be responsible for triaging referrals, undertaking assessments, reviewing care needs, and liaising with other health providers, families, and carers. There will also be an option to deliver nursing care and support in a patient-facing role to support business continuity.

As a senior nurse, you will contribute to service delivery development and project initiatives as agreed with the team lead. You will lead and participate in appropriate operational groups to ensure the delivery of the HHWC Strategy.

You will support the Home Nursing Services Team Leader and Deputy Team Leader to ensure continued service delivery and development, including fostering a skilled, effective workforce.

You will work under the direction and supervision of the Home Nursing Services Team Leader and Deputy Team Leader to ensure continued service delivery and service development.

Job responsibilities

JOB SUMMARY:

The role of the Senior Registered Nurse is to coordinate and manage the caseload of patients receiving home nursing and support at home services. This involves undertaking home assessments, assessing care needs, triaging referrals, reviewing care needs, and liaising with the hospice nursing team and other health and social care professionals, including the Continuing Health Care Team.

The Support at Home service (Domiciliary Care & Supported Living) is delivered by Hospice at Home West Cumbria through a commissioned service from the North Cumbria Clinical Commissioning Group (CCG).

There will be an option to deliver nursing care and support in a patient-facing role to support business continuity.

DUTIES AND RESPONSIBILITIES OF THE POST

Key responsibilities include:

  • Contributing to service development and project initiatives, leading and participating in operational groups to ensure delivery of the HHWC Strategy.
  • Supporting the Team Leader and Deputy Team Leader to ensure continued service delivery and workforce development.
  • Prioritising and coordinating daily care arrangements, ensuring appropriate staff deployment and high standards of communication with families and carers.
  • Assessing, planning, and implementing individualized patient care plans, regularly re-assessing care needs.
  • Administering medication for symptom management as required, including oral and subcutaneous medications and syringe drivers.
  • Participating in review meetings with the Continuing Health Care team to discuss support/care needs.
  • Supporting the on-call team rota, including planning and cancelling care, and liaising with out-of-hours professionals.
  • Monitoring care standards provided by home nurses, healthcare assistants, and volunteers.

Additional duties include reviewing assessments, supporting staff and volunteers, recruitment, maintaining accurate records, supporting audits and policy updates, and ensuring compliance with legal and professional standards.
