We are looking for a Senior Registered Nurse who can think on their feet and respond to rapidly changing situations. In this key role, you will support the development of a skilled, effective workforce and coordinate and manage the caseload of patients receiving home nursing and support at home services. As a proactive member of the Home Nursing Services team, you will be responsible for triaging referrals, undertaking assessments, reviewing care needs, and liaising with other health providers, families, and carers. As a senior nurse, you will contribute to service delivery development and project initiatives as agreed with the team lead. You will lead and participate in appropriate operational groups to ensure the delivery of the HHWC Strategy.
You will support the Home Nursing Services Team Leader and Deputy Team Leader to ensure continued service delivery and development, including fostering a skilled, effective workforce.

JOB SUMMARY:
The role of the Senior Registered Nurse is to coordinate and manage the caseload of patients receiving home nursing and support at home services. This involves undertaking home assessments, assessing care needs, triaging referrals, reviewing care needs, and liaising with the hospice nursing team and other health and social care professionals, including the Continuing Health Care Team.
The Support at Home service (Domiciliary Care & Supported Living) is delivered by Hospice at Home West Cumbria through a commissioned service from the North Cumbria Clinical Commissioning Group (CCG).
There will be an option to deliver nursing care and support in a patient-facing role to support business continuity.
Key responsibilities include:
Additional duties include reviewing assessments, supporting staff and volunteers, recruitment, maintaining accurate records, supporting audits and policy updates, and ensuring compliance with legal and professional standards.
Commitment to ongoing personal and professional development, mandatory training, and staying updated with policies, developments, and revalidation requirements.
Align personal conduct with the hospice's values, maintain confidentiality, promote the hospice's reputation, adhere to health and safety policies, and support equality and diversity initiatives. Undertake other duties as required.
