Job summary

You will oversee delivery of high-quality services, participate in rotating shifts from 7 am to 10 pm, and support clients post-hospital discharge. This role offers a hybrid working model and a clear path for NVQ progression.

Qualifications

• NVQ Level 3 or equivalent substantial experience in a related field.
• Experience in care or reablement settings is desirable.
• Rotating schedule including early starts is expected.

Responsibilities

• Monitor, support and supervise Support Workers to ensure high-quality services.
• Collaborate with occupational therapists to co-develop reablement plans.
• Deliver care in customers' homes following personalised plans.
