Job summary

Liquid Personnel is recruiting an experienced Advanced Practitioner (Social Care) – Children with Disabilities Team for its client's Children with Disabilities Team based in Halton. This is a full-time agency role offering £40.00 per hour UMB and supporting children and young people with disabilities through high-quality assessment, safeguarding, and care planning.

Qualifications

• Social Work England registration is required.
• Qualified Social Worker status with experience in Children's Services.
• Experience in Children with Disabilities Team preferred.

Responsibilities

• Manage a complex caseload of children and young people with disabilities.
• Complete assessments, care planning, and reviews per statutory responsibilities.
• Provide guidance, support, and mentoring to Social Workers within the team.
• Collaborate with families, health professionals, education services and partner agencies.
• Contribute to safeguarding, child protection and permanency planning as needed.
• Support service development initiatives and ensure high-quality case recording.
