Job summary

• A recognised Social Work qualification
• Strong knowledge of current adult social care legislation and guidance
• Experience undertaking assessments, reviews, and care management responsibilities
• Excellent communication and partnership-working skills
• A full UK driving licence
• A commitment to delivering high-quality, person-centred practice

Responsibilities

• Manage a varied caseload and undertake a broad range of adult social work duties
• Complete Care Act Assessments and Reviews
• Conduct Person-centred Support Planning
• Perform Mental Capacity Assessments
• Lead Section 42 Safeguarding Enquiries and CHC/Section 117 work
• Foster partnership working with health professionals and external agencies
• Promote independence through a strengths-based approach
• Support adults in the community to access the right services and networks
