Are you a dynamic, enthusiastic and experienced Band 6 clinician looking to work as a care co-ordinator? Then look no further! Come join our team in Basingstoke CMHT Meadows.
This role is to manage a caseload of patients through recovery to discharge, signposting back to the community.
Working 9-5pm (Can be flexible hours). Regular supervision and opportunities off training and further education.
A chance to develop your skill set and share your knowledge and experience with the team.
To act as a Care Co-Ordinator and support patients in the recovery. To ensure patients can be at the heart of their own care planning, ensuring collaborative care plans and risk assessments. With close working with carers, service partners and the voluntary sector.
To assess patients who have been referred to the CMHT.
To work as part of an MDT to ensure the most appropriate care.
