Who are we:
Holes Lane Medical Centre is a progressive, innovative, adaptive, and high achieving training practice established in Woolston, Warrington. We have 2 branches located within easy reach of the M62 and M6 motorways that serve a population of 10,200 patients.
We continually strive to, not only respond to the community's needs, but encourage the growth and development of all our staff. We believe that effective teamwork, collaboration, and continual improvement delivers strategies that keep the practice resilient. We are searching for a likeminded individual who shares this ethos.
In April 2024, we introduced My Care. A new approach in the management of care for patients with long-term conditions. It is collaborative, empowering the patient to have a quality of life by addressing their needs holistically and promoting shared decision making with the support of the clinician. Feedback from patients and staff has been very good. Continual nurse training will be offered to keep this initiative a success.
Job Description:
We seek an enthusiastic and experienced Senior Practice Nurse to join our dynamic team. This motivated and proactive individual must possess the relevant skills in Diabetes management and ideally hold prescribing qualifications. They will be expected to engage fully with the processes that maintain high QoF performance and deliver evidence-based clinical care. This individual would be expected to uphold the values of the practice described above.

Qualifications and Skills:
Essential
Proficient in diabetes management, with strong knowledge of current best practices.
Excellent communication and teamworking skills.
Proficiency with electronic patient record systems, ideally Systemone (TPP).
Competence with Microsoft Word and Excel.
Commitment to continuous professional development and evidence-based practice.
