Job summary

Locala Health and Wellbeing in West Yorkshire is seeking a Senior Physiotherapist to join the Urgent Community Response Team on a permanent basis. The role covers assessments, short-term interventions, and preventing hospital admissions while delivering care in the community.

You will work 37.5 hours per week, Band 6 (£39,959-£48,117), and travel as needed to meet service requirements. The post requires eligibility to work in the UK and a commitment to high-quality, patient-centered community

Qualifications

• Provide comprehensive assessment, treatment, and evaluation of patients referred to the service during periods of crisis.
• Deliver short-term intervention programmes aimed at stabilising patients and preventing deterioration.
• Manage a caseload of adults aged 18+ with complex health and social care needs in the community.
• Travel independently to meet service needs and demonstrate the right to work in the UK.

Responsibilities
