Job summary

Qualifications

• HCPC registration required for the Physiotherapist role.
• Registered Allied Health Professional with degree level or equivalent.
• Proven ability to work autonomously within community settings.

Responsibilities

• Undertake Occupational Therapy / Physiotherapy assessments and develop individualised treatment plans.
• Provide expert clinical advice and therapy to community patients at risk of deterioration or hospital admission.
• Maintain up-to-date records and activity data in line with standards.
• Deliver intervention in diverse environments (home, community, schools, workplaces).
• Supervise and support junior staff and contribute to service development.
