Would you like to work in a dynamic, ground-breaking, and fully integrated intermediate care team, in a brand new, purpose-built intermediate care hub? Due to re-organisation and restructure in the integrated service, we have Band 6 opportunities for Physiotherapists. Working to a home first agenda, the role is to support service delivery across inpatient, emergency department and community settings. Our service consists of Health and Social Care colleagues. Consultants, GPs, Advanced Care Practitioners, Social Workers, Nursing, OT, Physiotherapists, Discharge Team and Multi-Disciplinary Assistants all work together to support patient flow. We operate 7 days, 365 days a year to support some of our most vulnerable, frail patients. You will be based across our 4 community bases in addition to our new purpose-built Hub at Harrogate Hospital, working with colleagues from social care and our Acute Therapy Team.
