The role of Senior Physical Health Clinic Support Worker will be carrying our annual physical health checks for people diagnosed with a Serious Mental Illness (SMI) by conducting a range of physical observations and health promotion interventions, as well as signposting individuals to services to improve their physical health and set goals with individuals to help improve their physical health.
The Senior Physical Health Clinic Support Workers will also support long-acting antipsychotic injection clinics held at Cavendish House and Bexhill Health Centre by supporting the Qualified Nurse by completing administrative tasks such as note taking and arranging appointments (covered on a rota basis). Also, there is a supportive role within the weekly Denzapine clinic (covered on a rota basis).
Senior Physical Health Clinic Support Workers will also hold physical health clinics to conduct BMI assessment, ECG's and various blood tests as requested using venepuncture skills. (These assessments may also be undertaken in the community, as needed).
Other duties will include supporting the duty function of the Assessment and Treatment Team, as well as other duties relevant to the role and banding of this job.
We are looking for an individual who feels a desire to make a positive contribution to improving the physical health for people living with SMI within primary and secondary services and support them in accessing appropriate level of care.
You will be required to have strong organisational skills and be able to work independently within the boundaries of your role whilst being able to seek support and guidance from senior colleagues and other professionals (Consultants, nurses, VCSE organisations, social prescribers GP's and others). You will be working closely with the PCN surgeries, and the role requires the individual to be able to independently travel across the locality.
