Job summary

University Hospitals Dorset is seeking a dynamic Band 7 Clinical Practitioner to join the Perioperative Care of Older People (POPS) team at Royal Bournemouth Hospital. You will provide senior clinical assessment and expert input across the surgical pathway, working with surgeons, anaesthetists, geriatricians, therapists and discharge coordinators to optimise care for older patients.

The role requires autonomous practice, excellent assessment, decision-making, leadership and communication skills,

Qualifications

• Degree Level Qualification
• Level 6 or equivalent ECP experience
• Current registration with relevant professional body

Responsibilities

• Undertake comprehensive clinical and frailty assessments of older surgical patients.
• Contribute to preoperative assessment, shared decision-making and perioperative planning.
• Provide senior input into the assessment and management of frailty, delirium, falls, polypharmacy and functional decline.
• Develop and implement individualised discharge plans in partnership with the wider MDT.
• Provide continuity of POPS input throughout the surgical admission where appropriate.
• Support education, supervision and professional development of nursing, medical and AHP colleagues.
• Contribute to POPS audit, quality improvement, service development and the implementation of evidence-based practice.
