Job summary

Sanctuary Personnel are seeking a Band 6 Occupational Therapist to join the Acute Hospital Discharge Team in Sunderland. This ongoing locum role offers full-time hours with a competitive hourly rate of up to £26.23.

You will assess, plan and implement rehabilitation to support safe hospital discharges and maximise patient independence. You will work within a busy acute setting, collaborate with a multidisciplinary team, and contribute to service improvement while maintaining high standards of

Qualifications

• Qualified OT with HCPC registration.
• Experience in acute hospital discharge pathways.
• Excellent communication, written and verbal.
• Ability to manage a busy caseload and prioritise work.
• Commitment to CPD and evidence-based practice.

Responsibilities

• Undertake comprehensive OT assessments and deliver rehabilitation for complex needs.
• Lead discharge planning to ensure safe transitions from hospital to home or alternative settings.
• Manage a specialist caseload in acute/physical health settings.
• Apply advanced clinical reasoning and evidence-based practice.
• Collaborate with doctors, nurses, therapists and social workers to coordinate care.
• Provide advice and education to patients, families and carers.
