Job summary

South Gloucestershire Council is seeking a Senior Practitioner in the Promoting Independence and Reablement team to lead assessments, care and support planning, and service provision for adults across the authority.

You will provide professional guidance and oversight for referrals from the community and discharge pathways, supervise practitioners, support development of students and newly qualified staff, and work in partnership with reablement providers and health partners.

Qualifications

• It is essential to hold HCPC registration with a relevant professional qualification.
• Significant post-qualifying experience in a social care setting including complex case management.
• Ability to assess risk and provide clear guidance and supervision on casework.

Responsibilities

• Provide professional guidance and oversight for community referrals and reablement service.
• Lead day-to-day management of the team, with supervision and PDPRs.
• Assess risk, identify safeguarding concerns and direct complex cases.
• Maintain high performance and quality standards with statutory/local compliance.
• Produce clear, concise, high-quality reports and keep records up-to-date.
• Support development of students and newly qualified staff.
• Collaborate with reablement providers and health partners.
