Sussex Partnership NHS Foundation Trust is seeking a Band 7 Senior Nurse Practitioner to join the Mental Health Liaison Team in Eastbourne. The role concentrates on patient flow, discharge from inpatient and A&E, and engagement with high-intensity and complex relational presentations to improve urgent care pathways.

You will lead audits, supervise junior staff, review policies, liaise with partner organisations, and apply psychosocial interventions including psycho-education and discharge

Formación

• Registered Nurse with current UK registration in mental health/LD/adult.
• Experience in urgent and acute care settings and complex presentations.
• Evidence of post-registration training in relevant service area.
• Ability to supervise and mentor junior staff and students.
• Commitment to risk management and multidisciplinary collaboration.
• Right to work in the UK and eligibility to practice in NHS.

Responsabilidades

• Provide specialist assessments and nursing interventions to patients within the care group.
• Lead on discharge planning, patient flow and liaison with inpatient and A&E Eastbourne.
• Review clinical policies and implement changes to practice.
• Liaise with partner organisations to promote communication and good practice.
• Undertake audits and provide supervision to staff in the service area.
• Deliver psychosocial interventions including psycho-education and recovery plans.
