Sussex Partnership NHS Foundation Trust is seeking a Mental Health Practitioner to join the Lewes Neighbourhood Mental Health Team. The role focuses on triage, assessments, and brief interventions, with liaison across GP practices, NHS Talking Therapies and VCSE partners.

The post offers a supportive environment, extensive professional development and opportunities to contribute to service improvement and triage hub operations in East Sussex.

Formación

• Registered Mental Health Professional (Nurse, Social Worker or Occupational Therapist) is essential.
• Experience in triage, assessment and brief interventions is required.
• Strong communication and liaison skills with GP practices and VCSE partners.

Responsabilidades

• Triage referrals into the NMHT and NHS Talking Therapies pathways.
• Carry out timely and consistent assessments and brief interventions.
• Signpost and refer to Secondary or Urgent Care as needed and coordinate with partner services.
