Job summary

NHS Cheshire and Merseyside Integrated Care Board is seeking a Band 6 Senior Individual Assessment and Commissioning Nurse to shape and commission care packages for complex cases. The role covers NCA across multiple areas within Cheshire & Merseyside, offering a structured, predictable schedule and supportive team.

The post requires a registered nurse with strong assessment, decision-making and partnership skills, delivering person-centred care and leading on MDT collaboration and performance

Qualifications

Registered nurse (Adult, Mental Health, or Learning Disabilities).
Strong clinical assessment and decision-making skills.
Experience with caseload management and coordination.
Current Nursing registration and CPD evidence.

Responsibilities

Lead on assessment and case management.
Commission safe, effective, and tailored care packages.
Ensure timely, person-centred, outcome-focused care.
Collaborate with patients, families, providers and MDTs.
Navigate national policy, guidance, and legislation.
Support safe hospital discharges and transitions of care.
