Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust in Southampton is seeking a Senior Neuro Occupational Therapist to join the Community Neuro Rehabilitation Team at Adelaide Health Centre. You will manage a caseload and work within a multidisciplinary team to support transitions from children to adult services.

This role requires neuro-specific experience, a willingness to lead on service improvements, and occasional community travel. Closing date 15 November 2026.

Qualifications

• Degree or Diploma in Occupational Therapy, state registered.
• Post-registration courses include Stroke Rehabilitation, LTC management, Palliative care, Postural management, Continence Care.

Responsibilities

• Work in a multi-professional team, coordinating therapeutic input for neuro conditions.
• Manage a caseload and coordinate transition-focused input with health and social care professionals.
• Provide guidance on the transition process to health and social care staff and families.
• Collaborate with the Service Matron to improve care pathways and processes for seamless transitions.
• Assess and treat clients with complex/chronic presentations requiring specialist rehab.
• Plan and implement comprehensive intervention programmes.
• Organise the team’s caseload and duties in the absence of the team leader; attend MDTs and advise colleagues.
• Supervise junior staff and students; contribute to service developments.
