Caredig in Swansea is seeking a Senior Mental Health Support Worker to provide high-quality, person-centred support and help people gain greater independence. You will act as a Key Worker, delivering psychologically informed support, assisting with daily living skills, appointments, and community engagement while promoting dignity and choice.

Qualifications

• Experience supporting people with mental health, substance misuse, housing or complex support needs.
• Experience developing person-centred support plans and helping people achieve positive outcomes.
• Strong communication and relationship-building skills.
• Good IT and record-keeping skills.
• Full UK driving licence.

Responsibilities

• Act as a Key Worker and support people to achieve their personal goals.
• Deliver person-centred, psychologically informed support.
• Support with daily living skills, medication, appointments and community engagement.
• Promote independence, wellbeing, dignity and choice.
• Maintain accurate records and facilitate support reviews.
• Work flexibly across services, including evenings, weekends, nights and sleep-ins where required.
