The role requires mobility, a hybrid working model, and delivering patient-centred mental health assessments and interventions in line with trauma-informed practice, supervising junior staff and coordinating care with families and the MDT.

Responsibilities

• Lead holistic assessments and safety planning for veterans with mental health needs.
• Develop Personal Wellbeing Plans in collaboration with service users and families.
• Coordinate care across the MDT and act as keyworker for patients.
• Provide mentorship and supervision to Band 5 nurses and junior staff.
• Ensure accurate documentation and continuity of care.
