Job summary

Surrey and Borders Partnership NHS Foundation Trust is seeking a Senior Mental Health Practitioner to join the Brief Interventions pathway, delivering person-centred care to adults with established mental health needs in the community. The role is full-time (37.5 hours over five days).

You will work closely with GP practices and the primary care team to provide assessments, brief interventions and care navigation.

Responsibilities

• Manage a complex caseload and undertake comprehensive assessments.
• Deliver psychosocial interventions and care navigation in the community.
• Work closely with General Practitioners and primary care teams to provide timely care.
• Collaborate with service users, carers, and partner agencies to ensure coordinated care.
