• Demonstrated ability to assess and manage mental health risk.
• Experience working in community settings and GP surgeries.
• Ability to lead professional care for complex needs.
• Strong collaboration with patients, carers, families and GPs.

• Undertake comprehensive mental health assessments and risk assessments.
• Deliver evidence-based nursing interventions and treatment plans.
• Act as a Lead Professional for individuals with complex mental health needs.
• Work collaboratively with patients, carers, families, and partner agencies and GPs.
• Promote recovery-focused and person-centred care.
• Contribute to safeguarding, risk management, and clinical governance requirements.
• Support service development and quality improvement initiatives.
• Demonstrate good communications skills and ability to work in a multi-agency team.
