Job summary

Leicestershire Partnership NHS Trust (LPT) is seeking Mental Health Practitioners (Band 6) to join the Assertive Outreach Service across Leicester, Leicestershire and Rutland. The post requires a Mental Health Nursing qualification and involves autonomous work within a multidisciplinary team delivering intensive community care.

You will assess, plan, deliver and evaluate care, take a key worker role for complex cases, and liaise with partners to coordinate services.

Qualifications

Excellent assessment, risk management, organisational and communication skills are essential.
Experience with care coordination policies and CTOs is beneficial.
Ability to work autonomously as a key worker for complex cases within home-based packages.

Responsibilities

Assess, plan, deliver and evaluate care for service users referred to the Assertive Outreach Service, including risk assessment and management.
Act as a Key worker for a caseload of service users with complex needs, developing therapeutic relationships.
Provide information to clients and carers about prescribed medications and carry out specialised assessments within your registration.
