Job summary

Leicestershire Partnership NHS Trust is seeking a Mental Health Practitioner (Band 6) to join the Assertive Outreach service, delivering and evaluating care for service users in their own homes as part of a multidisciplinary approach. This role supports people with enduring mental health problems who struggle to engage with traditional services.

Applicants must be a Mental Health Nurse and commit to working across Leicester, Leicestershire and Rutland, using risk assessment, care coordination

Qualifications

• Excellent assessment, risk management, organisational and communication skills.
• Experience with Care Co-ordination policy and CTOs.
• Ability to work autonomously and liaise with other agencies.

Responsibilities

• Assess, plan, deliver and evaluate care for service users referred to the Assertive Outreach Service.
• Undertake the role of Key worker for a caseload of service users with complex mental health problems.
• Provide information on prescribed medications and devise plans of care in line with registration requirements.
