Job summary

West London Mental Health Trust is seeking a Band 6 Community Mental Health Nurse to join the Assertive Outreach Team in Hounslow. The role involves autonomous, hands-on clinical work with individuals with complex psychosis, delivering assessments, care planning and risk management in community settings.

You will operate as a Named Worker within a multidisciplinary team, coordinating with crisis services, police, housing and other agencies to provide recovery-focused, supported community care.

Qualifications

• Experienced in working with people with severe and enduring mental illness, particularly psychosis.
• Confident in clinical assessment, risk formulation and management, including risks to others.
• Ability to work independently while seeking advice when needed.
• Persistent, flexible and creative in engaging reluctant service users.
• Solid understanding of mental health legislation, safeguarding and medicines management.
• Previous experience in Assertive Outreach is welcome but not essential.

Responsibilities

• Undertake assessments and develop care plans as a Named Worker within a multidisciplinary team.
• Deliver interventions that promote recovery and sustained engagement.
• Manage clinical risks and make sound decisions in complex presentations.
• Collaborate with service users, families, carers and partner agencies to provide coordinated care.
