We are an integrated team working with wards and day-care facilities as a partial hospitalisation service, enhancing patient-centred care and ensuring smooth transitions across services for high-quality patient and carer support.

In this role, you will carry out emergency assessments from Community Teams, GPs, and A&E Out of Hours to gate-keep admissions, coordinate ongoing care, or refer to external teams. The team also visits Service Users at home to plan and implement care packages for Crisis and Home Treatment Team patients.
