Job summary

Oxford Health NHS Foundation Trust is seeking a Specialised Mental Health Case Manager to join the Thames Valley CAMHS Provider Collaborative on a maternity cover until January 2028. You will work to improve clinical care across Oxfordshire and surrounding areas, coordinating care with ICBs and partner organisations.

The role requires post‑graduate knowledge of mental health services, robust case management skills and the ability to chair reviews.

Qualifications

• Detailed knowledge of mental health services and legislation relevant to specialist care.
• Post-graduate level experience in senior mental health roles or equivalent.
• Willingness to engage with peers and professionals to deliver appropriate interventions.

Responsibilities

• Provide clinical assurance of care quality within the Thames Valley region.
• Support compliance of specialised services with guidance including Transforming Care.
• Coordinate with Integrated Care Boards to ensure coordinated care and discharge planning.
• Monitor patient pathways to achieve high-quality services and outcomes.
• Oversee the overall patient pathway and promote least restrictive care and timely discharge.
• Chair and oversee inpatient Care, Education and Treatment Reviews for people with learning disabilities or autism.
