Job summary

Central London Community Healthcare NHS Trust is seeking a Senior Medicines Optimisation Pharmacist to lead the Bedded Services team within Medicines Management, delivering a high-quality clinical pharmacy and medicines optimisation service for inpatients in Community Hospitals.

The role includes staff leadership, clinical governance, and working across CLCH sites. GPhC registration and postgraduate clinical pharmacy qualifications are essential.

Qualifications

• Post-qualification clinical pharmacy experience in hospital settings.
• Experience in medicines management initiatives and clinical governance.
• CPD evidenced by portfolio and reflective practice.

Responsibilities

• Deliver a high-quality clinical pharmacy and medicines optimisation service to inpatients in Community Hospitals.
• Ensure continuity of pharmaceutical care for patients admitted from primary and secondary care.
• Lead, manage and develop staff in the Bedded Services team.
• Contribute to clinical governance and national/local policy alignment.
• Work across CLCH locations and other sites as required.
