Job summary

NHS Jobs in London seeks an Accredited Checking Pharmacy Technician (ACPT) to support Private Care medicines management and accuracy checks. You will work with pharmacists and the broader team to ensure safe, effective medicines delivery and patient support.

You will engage in medicines reconciliation, data collection, audits and CPD, and may cover weekend/late shifts. Cross-site coverage at Chelsea and Cavendish Square is included as needed.

Responsibilities

• Work closely with the wider pharmacy teams to ensure high-quality medicines management in Private Care.
• Undertake accurate medicines reconciliation and histories, resolving medicines-related issues with professional judgement.
• Provide patients, carers and healthcare professionals with clear information to support safe medicines use and adherence.
• Prioritise and organise workload to respond to clinical and operational demands while maintaining accuracy.
• Act as Accredited Checking Pharmacy Technician (ACPT) for final accuracy checks of dispensed medicines.
• Collaborate with Pharmacists and multi-disciplinary teams to optimise medicines management services.
• Assist lead Pharmacist to review and improve patient experience within Private Care.
• Contribute to medicines management training, accreditation and checking programs.
• Participate in clinical audits and data collection activities.
• Report incidents/near misses to mitigate risks and support investigations.
• Assist in recruitment and selection of pharmacy staff when appropriate.
• Participate in pharmacy initiatives as needed.
• Take part in 7-day, weekend and late duty rotas when requested.
• Provide cross-cover at Chelsea and Cavendish Square sites as required.
• Carry out other duties consistent with the post’s nature as may vary.
• Maintain a CPD portfolio and continue education.

Job description

• 2.6. Work closely with the wider pharmacy teams to ensure a seamless high-quality delivery of effective medicines management service to the clinical services within Private Care.
• 2.7. Undertake accurate medicines reconciliation and medicines histories, identifying and resolving medicines-related issues, exercising professional judgement and referring clinical concerns to the Pharmacist or appropriate clinician.
• 2.8. Provide patients, carers and healthcare professionals with clear, appropriate information to support safe medicines use and adherence.
• 2.9. Prioritise and organise your workload effectively, responding to changing clinical and operational demands while maintaining a high standard of accuracy.
• 2.10. To work as an Accredited Checking Pharmacy Technician (ACPT) to undertake final accuracy checks of dispensed medicinal products.
• 2.11. Work closely with the clinical Pharmacists and wider multi-disciplinary team to undertake projects and pathway management to optimise medicines management services.
• 2.12. Work with the lead Pharmacist for Private Care to review and implement positive changes to patient experience within Private Care.
• 2.13. To assist in the delivery of medicines management training, accreditation and accredited checking programs.
• 2.14. To actively participate in clinical audit and data collection.
• 2.15. To actively report incidents/near misses, to mitigate future risks and when appropriate to support in the investigation of incidents, identify root cause to establish plans to mitigate future risks in a timely manner.
• 2.16. Assist with the recruitment and selection of Pharmacy staff as appropriate.
• 2.17. Participate in pharmacy initiatives as appropriate.
• 2.18. Participate in the 7-day, weekend and late duty rotas as requested.
• 2.19. To provide occasional cross cover when required at Chelsea and Cavendish Square sites.
• 2.20. To carry out any other duties consistent with the nature of the post, which may vary from time to time.
