You Matter in Liverpool is seeking a Senior Housing Support Practitioner to support residents moving towards independent living in the community. The role involves managing a large caseload, focusing on resettlement, wellbeing, tenancy sustainment, and building therapeutic relationships.

You will work across Nightingale House sites, undertake health and safety duties, lone working, and contribute to service leadership with the Nightingale Leadership Team.

Qualifications

• Experience with mental health, homelessness and substance misuse.
• Ability to lead practice and manage a large caseload.
• Experience of safeguarding and partnership working.
• Experience in resettlement and independent accommodation.
• Skills in risk assessment and safety management.

Responsabilités

• Manage a large caseload and provide person-centred support.
• Oversee health and safety and safeguarding within the service.
• Support lone-working and maintain a visible presence at Nightingale House.
• Participate in leadership meetings and reflective practice.
• Coordinate outreach, engagement with professionals and other providers.
