  • Providing guidance and support to Home Support Assistants working within the community.
  • Working collaboratively with health and social care professionals as part of an integrated care approach.
  • Supporting people to regain and maintain independence through rehabilitation and enablement activities. Monitoring service users' wellbeing and reporting significant changes or emerging risks.
  • Completing risk assessments and maintaining accurate electronic records. Promoting dignity, choice, privacy and person-centred care at all times.
  • Supporting safe medication practices and ensuring compliance with service procedures.
  • Contributing to safeguarding adults by recognising and responding appropriately to concerns.
  • Assisting managers with operational responsibilities and promoting high standards of care delivery.
About You

You will bring:

  • Ability to travel across the Sandwell borough.
  • Applicants must be aged 18 years or over.
