Job summary

Channel Health Alliance is seeking an experienced Integrated Neighbourhood Team GP to provide senior clinical leadership across the PCN. You will mentor clinicians, supervise triage decisions, and oversee care for housebound patients in the community.

The role involves autonomous practice, collaboration with GP practices and community services, and delivering high-quality, patient-centered care within a dynamic team. Interview date: 21 October 2026.

Qualifications

• Essential: Medical Degree
• Essential: Full GMC Registration
• Essential: Registered on the Performers List
• Essential: Applicants must have current UK professional registration.

Responsibilities

• Provide senior clinical triage and determine the most appropriate response to home visit requests.
• Deliver clinical leadership, mentorship, and support to Advanced Clinical Practitioners and the wider multidisciplinary team.
• Undertake home visits for patients with complex medical needs when required.
• Assess, diagnose, and manage patients in their own homes.
• Prescribe medication according to national and local prescribing guidance.
• Carry out CGA, RESPECT and SMRs.
• Support service improvement, education, training, and clinical governance activities.
• Work collaboratively with GP practices, care homes, community services, and partner organisations.
• Ensure high standards of patient safety, record keeping, and professional practice.
