South Warwickshire University NHS Foundation Trust is recruiting a Consultant to support and develop the Acute Frailty Service at Warwick hospital. The role entails comprehensive geriatric assessment of frail older adults in crisis, with on-call GIM responsibilities and close collaboration with emergency, acute medicine and community teams.

The post is open to applicants with Geriatrics and General Internal Medicine CCT or within 6 months of expected CCT completion, and comes with a full-time

Qualificações

• Eligible for the specialist register in Care of the Elderly (with general medicine) or within 6 months of CCT Date
• Desirable: Demonstrable commitment to teaching and mentoring of medics in training and other MDT members.

Responsabilidades

• Lead a multidisciplinary team across the Frailty Assessment Area, Acute Frailty Ward, Acute Medicine, Emergency Department and Community services (UCR).
• Support the evolution of the Acute Frailty service through new technologies, capacity expansion, data collection and workforce development.
• Provide care for frail older adults accessing urgent care at the Trust and contribute to a cohesive, innovative team.
