Job summary

South West London and St George's Mental Health NHS Trust in London is seeking an Enhanced Clinical Practitioner for the Wandsworth Older People Community Team. Qualifications

• Extensive older adult practice-based experience aligned to the four pillars of advanced practice (Clinical Practice, Leadership, Education and Research).
• Ability to lead clinical service development and quality improvement initiatives within older people services.
• Strong collaboration with multi-disciplinary teams, patients and carers to deliver person-centred care.

Responsibilities

• Act as a key worker for an identified caseload of patients with complex needs while balancing clinical leadership and quality of care.
• Undertake comprehensive history taking, assessment and formulation from biological, psychological, social and spiritual perspectives.
• Prioritise interventions in complex, changing or high-safety-risk situations and contribute to care planning modifications.
• Lead on delivery of high quality, safe and contemporary care and participate in research and quality improvement initiatives.
