Wight Healthcare invites applications for a Band 6 Community Mental Health Practitioner based at Parklands House, London Road, Basingstoke. The Older Persons CMHT provides assessment, diagnosis, treatment and support for people with dementia or functional mental health illnesses over 65.

You will carry a caseload, work within a multidisciplinary team, supervise staff, run clinics as needed, and coordinate complex care while producing reports and maintaining documentation.

Formación

• Experience in senior mental health care for older adults.
• Proven ability to assess and manage complex cases.
• Excellent communication and report writing skills.
• Driving licence and access to vehicle in the UK.

Responsabilidades

• Lead assessment, diagnosis and treatment planning.
• Coordinate complex care and act as care coordinator.
• Supervise staff and run clinics as required.
• Produce reports and maintain documentation.
• Participate in duty rotas and safeguarding duties.
