Job summary

Blackpool Teaching Hospitals NHS Foundation Trust in Kirkham is seeking a Senior Community Nurse (Band 6) to join the Kirkham District Nursing Team. The role requires proactive leadership, expert community nursing, and strong partnership working within the NHS.

The post involves 37.5 hours per week in a 7-day service with a rota, managing a district caseload and providing high-quality care in patients’ homes. Driving licence and car access are essential.

Qualifications

• Registered Nurse level 1 and current NMC registration.
• Prescribing qualifications V100/V150/V300.
• Evidence of ongoing professional development.
• Supervisor/Assessor competence.
• CSP qualification and degree in nursing.

Responsibilities

• Holds delegated 24 hr continuing responsibility for a caseload and coordinates admissions and discharges.
• Provide clinical leadership and triage support on a rota basis.
• Assess, plan, implement, evaluate, and reassess home care for patients.
• Lead district nurse team and supervise staff.
• Collaborate with health and social care, voluntary and private sectors.
• Contribute to development of the nursing service.
• Organise community schedule for students and provide induction training.
