Job summary

Blackpool Teaching Hospitals NHS Foundation Trust is recruiting a Senior Community Nurse (Band 6) for the Kirkham District Nursing Team. The role requires leadership in a 37.5-hour week across a 7-day service. Driving licence and car access are essential to visit patients in the community.

You will provide high-quality nursing care, oversee clinical governance, and liaise with other services to ensure effective community care delivery. A DBS check and NHS values alignment will apply.

Qualifications

• Registered Nurse with current NMC registration and drive for ongoing professional development.
• Prescribing qualifications (V100/V150/V300) and ability to work within prescribing protocols.
• Degree in Nursing and evidence of continuing professional development.
• Able to lead and develop the district nursing team in the community.
• Experience in community nursing and care planning.

Responsibilities

• Assess, plan and evaluate care for patients in the community and coordinate caseloads.
• Provide clinical leadership and supervise district nursing staff.
• Prescribe in line with Trust guidelines and relevant policies.
• Maintain accurate clinical records and documentation.
• Collaborate with partners across health and social care to deliver services.
