Job summary

Calderdale and Huddersfield NHS Foundation Trust is seeking a part-time Rehabilitation Assistant (Band 3) to provide community-based support across Calderdale and the Halifax area. You will deliver therapy- or care-related programmes at patients’ homes, promote independence and coordinate with a multidisciplinary team to support safe discharges and timely interventions.

Based in the UCR, this role offers flexible part-time hours, with a pro-rated salary of £25,760–£27,476 per year.

Qualifications

• English and Maths GCSE grade 9-2 (A*-E) or equivalent demonstrated literacy/numeracy.
• Competence-based Level 3 Qualification e.g. NVQ or Diploma in Care or The Care Certificate.
• Willing and able to participate in Competency Based Training Programme.
• Evidence of competency-based training in relevant field.

Responsibilities

• Work in patients' home to implement treatment to protocols.
• Monitor progress and provide feedback to the registered practitioner.
• Make minor adjustments to treatment programmes within predefined limits.
• Assist registered practitioners with complex treatment.
• Assess for, order and issue basic equipment per protocols and guidelines.
