Oxford Health NHS Foundation Trust is seeking an experienced Community Mental Health Nurse to join the Older Adult Community in High Wycombe. You will provide patient-centred home-based care within a supportive multidisciplinary team and manage a defined caseload.

The role offers training opportunities, supervision, and the chance to work with older adults in South Buckinghamshire, delivering treatments in community bases, clinics and patients' homes.

Qualifikationen

• Experience as a community mental health nurse is required or demonstrated ability to work with Older Adults.
• Ability to deliver home-based treatment and develop personalised care plans.

Aufgaben

• Independently assess patients' mental health needs and develop personalised care plans.
• Deliver home-based treatment and support for patients in the community experiencing mental health crises.
• Manage a defined caseload of patients as their key worker with ongoing assessment and plan adjustments.
• Collaborate with patients, carers, and other health professionals to set and achieve treatment goals.
• Provide therapeutic interventions and manage discharge processes for safe transitions.
• Engage in supervision, contribute to team training, and participate in service improvement initiatives.
