Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking an experienced Band 6 Physiotherapist to join the Fareham and Gosport Community Therapies Team. You will assess and treat adults in the community, manage your own caseload, and work within a supportive multidisciplinary team to maximise independence and rehabilitation goals.

Qualifications

HCPC registration as a Physiotherapist is essential.
Degree level or Diploma equivalent.
Access to a vehicle for work is required.
Experience in community/rehabilitation settings is desirable.

Responsibilities

Undertake Occupational Therapy/Physiotherapy assessments in the community with diverse needs.
Manage own caseload using clinical reasoning to develop treatment plans.
Maintain up-to-date records and activity data.
Provide interventions in clients' homes and community settings.
Supervise and support junior staff and colleagues as required.
Work collaboratively with multi-disciplinary teams to support safe, coordinated care.
