Job summary

North Cumbria Integrated Care NHS Foundation Trust in Maryport, Cumbria, is seeking a Senior Community Occupational Therapist (Band 6) on a full-time permanent basis to join the Maryport/Cockermouth ICC.

You will work within the Community Rehabilitation Team, supporting hospital discharge, preventing admissions, and delivering home and community inpatient care, with 37.5 hours per week, on a rota including weekends. Salary £39,959 - £48,117 per annum.

Qualifications

  • Assess adults/older adults using a range of standardised assessments and clinical observation skills.
  • Provide advice in formulation or progression of a medical diagnosis and support family through the process.
  • Formulate specialist treatment plans with advanced clinical reasoning and set goals.
  • Deliver specialist OT treatment interventions in home and community hospital settings.

Responsibilities

  • Undertake assessment and clinical diagnosis across physical, sensory, cognitive, emotional, psychological and social domains.
  • Provide expert OT input to support discharge planning and anticipatory care.
  • Deliver interventions to enable patients to achieve occupational goals in varied environments.

We are looking for an innovative and enthusiastic Occupational Therapist to join our supportive Community Rehabilitation Team working across the Maryport & Cockermouth ICC on the fringe of the beautiful Lake District.

You will work as part of our established team which supports hospital discharge, works to prevent hospital admission, offers anticipatory care as well as a community hospital inpatient ward.

This role will support patient's receiving the Home Care Practitioner service using models of occupational therapy you will assist individuals to achieve their occupational goals, sharing your skills and expertise with others. An interest in personalised care handling is required but training can be provided

This is a full-time post, with 37.5hours per week, for the right candidate.

Main duties of the job

To undertake assessment, clinical and functional diagnosis of adults/older adults using a range of specialist standardised assessments and clinical observation skills across physical, sensory, cognitive, emotional, psychological and social domains.

To provide advice in the formulation or progress of a medical diagnosis and to support family members through this process guided by specialist theoretical knowledge and enabled by establishing a therapeutic alliance.

Contribute to the MDT’s understanding of the family system and its impact on the patient, the patient’s present circumstances, and the impact of that on the family system.

To formulate agreed specialist individual and/or group treatment plans and goals based on advanced clinical reasoning skills with the adult etc, taking in to account the domains assessed.

To provide specialist occupational therapy treatment interventions in a variety of environments including home and community hospital bed setting

To provide spontaneous and planned specialist advice, teaching and instruction to family members, carers and other relevant professionals in health, to promote understanding of the aims of occupational therapy and to ensure a consistent and coordinated approach across the AHP service.

Interventions regularly include the assessment, evaluation and prescription of specialised equipment for the adults. Standards for offering specialist advice for its safe use and the aims and purpose of the equipment apply as above.
