Qualifications

• Registered nurse with current NMC registration and nursing degree or equivalent.

Responsibilities

• Provide effective evidenced based care to a defined population within the community setting.
• Assess, plan, implement and evaluate individualised patient care with professional judgment.
• Provide clinical leadership and line management to junior staff within the team.
• Contribute to continuous improvement of community nursing within the Place area.
