The West Essex Specialist Dementia and Frailty Service (SDFS) within Essex Partnership University NHS Foundation Trust is recruiting a clinician to provide care coordination for a caseload of complex patients and to support the daily Urgent Community Response rota. You will work with the MDT and multi‑agency teams to deliver high quality, personalised care, safeguard, and support under-65 assessments.

Job description

Job Overview

The West Essex Specialist Dementia and Frailty Service (SDFS) is an assessment, diagnostic and support pathway for people with dementia of any age, as well as people over the age of 75 who have complex mental health needs and an associated age related frailty.

We work closely with our EPUT community specialist practitioners and Essex County Council colleagues, local organizations as well as the voluntary sector to support the holistic needs of the patient with a person centred collaborative approach.

The successful candidate will be trustworthy, positive about change and will be passionate about the services we provide and have excellent interpersonal skills and a zest for delivering the highest level of patient care and service delivery improvements.

The successful candidate will have excellent communication and documentation skills, be organised and able to prioritise workloads, be flexible and have a 'can do' attitude, be able to work independently as well as alongside patients and their families, the ward and community based staff as a valued member of the team

The successful candidate will be able to adhere to the trust core values of being compassionate, open and empowering.

Main duties of the job
  • To provide care coordination to a caseload of complex patients
