Sirona care & health CIC is seeking a Community Specialist Practitioner – Long Term Conditions (CSP-LTC) at Band 7 for maternity cover. The role is part-time (26.25 hours per week) with flexible working and NHS benefits.

The post is based in Warmley within the South Gloucestershire/NS Bristol area of England. The CSP-LTC will autonomously identify and case-manage highly complex cases, work with PCNs and Integrated Network Teams to prevent avoidable hospital admissions, and provide clinical

  • Post-graduate L3 (or higher) course or equivalent modules.
  • Evidence of continuous personal and professional development.
  • Masters level clinical qualification or working towards it.

Responsibilities

  • Identify and case manage individuals with highly complex needs in the community.
  • Collaborate with Primary Care Network and Integrated Network Team to prevent admissions and facilitate timely discharges.
  • Provide clinical leadership and direction to the INT and Locality teams.
  • Support cross-cover of other Sirona services to ensure resilience.

Our Integrated Network Teams within South Glos (INTs) deliver out of hospital care to people in their own homes or community clinics within the area South Glos

Community Specialist Practitioner – Long Term Conditions (CSP-LTCs) are experienced, specialist clinicians with advanced clinical skills, case management techniques and a background in shared decision-making. They support people who are high intensity users of health care services. With this specialist intensive support, these service users are able to remain safely at home and have more choice about their health care.

Case management is key for the support of people with long term conditions. This role requires expert evidence based practitioner knowledge gained from extensive front line, clinical leadership, decision making and managerial experience

The Locality teams are made up of:

  • Community Specialist Practitioners Urgent and Long-term Conditions
  • Community Nurses
  • Community Therapists
  • Paramedics
  • Community Support Workers
  • Administrators
This is a fixed-term post to provide maternity leave cover within the South Gloucestershire INTs. Applications are therefore invited from individuals currently working within the South Gloucestershire INTs who have an understanding of local processes, systems, and ways of working.
Main duties of the job

The purpose of this fixed term role is to work autonomously as a specialist practitioner to identify and case manage individuals with highly complex needs who require intensive and community support. You will work with their Primary Care Network (PCN) in partnership with the Integrated Network Team (INT) and the primary care team to prevent avoidable admissions and facilitate timely discharges from hospital into the community setting and, where possible, improv their quality of life.

As an experienced practitioner you will provide a service that focuses on achieving and demonstrating overall improvements in health care for people who are frail and have several long-term conditions. You will also provide clinical leadership and direction to the INT, supporting the Locality Manager and Assistant Locality Manager to deliver excellent evidence based clinical outcomes. To provide organisational and service resilience, you may be expected to cross cover other teams or services, both within the locality and across Sirona services.

Qualifications
  • Current registration with NMC / HCPC or other relevant professional body of registration.
  • Successful completion of a relevant post-graduate L3 (or higher) course or appropriate equivalent modules, such as disease specific qualifications, physical assessment and clinical reasoning.
  • Evidence of continuous personal and professional development and ability to study at Masters level.
  • Willingness to undertake training required for the role
  • Masters level clinical qualification or working towards achieving this.
Skills, Knowledge & Experience
  • Knowledge of local, national, strategic and operational policy developments and an awareness of their implications
  • Understanding of the primary care, secondary care and provider interface.
  • Expert knowledge of complex case management
  • Experience of community and multidisciplinary team working
  • Ability to prioritise against tight deadlines and work under pressure
  • Experience of undertaking clinical audits and analysing their results.
  • Coaching / Teaching and Assessing qualification / experience.
  • Knowledge of legal issues relating to the care of people such as Data Protection Act, Adult Protection procedures and the Mental Capacity Act.
