Royal Surrey NHS Foundation Trust is seeking a Band 6 Community Mental Health Practitioner to join the Neighbourhood Mental Health Teams across Surrey and Northeast Hampshire. The role involves leading assessments, delivering CBT-based interventions, and collaborating within a multidisciplinary team to support service users in recovery.

Applicants must hold NMC RMN registration, a mentorship qualification, and a valid driving licence with access to a vehicle for community-based work.

Competenze

• NMC Registration RMN is required.
• Mentorship course completed.
• Driver’s licence with access to a vehicle for community visits.
• 2 years’ post-qualification experience.
• Postgraduate diploma level or 5 years equivalent experience.

Mansioni

• Lead clinical management of a care package for a defined group of service users.
• Co-ordinate and monitor delivery of person-centred healthcare with complex clinical decisions.
• Deliver individual and group therapy using CBT skills.
• Work within the MDT to assess and plan care and respond to changes in health.
• Ensure accurate recording of contacts in case notes and maintain effective communications.
