Job summary

Royal Surrey NHS Foundation Trust is recruiting a Band 5 Mental Health Practitioner to join Neighbourhood Mental Health Teams across Surrey and Northeast Hampshire. The role focuses on comprehensive assessments, evidence-based interventions, and care coordination within a multi-disciplinary team.

Qualifications

• NMC Registration RMN (UK) is required.
• Professional qualification with degree or equivalent.
• Judgements on problems requiring investigation and analysis.
• Clinical risk assessments within the scope of competence.

Responsibilities

• Undertake comprehensive mental health assessments and risk assessments.
• Deliver evidence-based nursing interventions and treatment plans.
• Act as a Care Coordinator/Lead Professional for individuals with complex mental health needs.
• Work collaboratively with patients, carers, families and partner agencies.
• Promote recovery-focused and person-centred care.
• Contribute to safeguarding, risk management and clinical governance.
• Support service development and quality improvement initiatives.
• Demonstrate good communications skills and work within a multi-agency team.
