Job summary

NHS Greater Glasgow and Clyde (NHSGGC) invites a Band 6 Community Nurse to join the South Glasgow Older People’s Mental Health Team. You will assess, plan and deliver care for adults 65+, working across Elderpark Clinic, Shawmill Resource Centre and Govan, in a multidisciplinary setting.

You will lead clinical care, mentor junior staff, participate in service development and potentially deputise for the Nursing Team Leader. Fixed term maternity cover, 36 hours weekly, Mon-Fri 9am-5pm.

Qualifications

• First level RMN registration.
• Minimum of 2 years' community mental health experience at Band 5 level or equivalent.
• Evidence of continuing professional development (CPD).
• Excellent communication and organisational skills.
• Current driving licence (Essential).
• Experience of working with older people with functional and organic mental health conditions.
• Ability to work effectively as part of a multidisciplinary team.

Responsibilities

• Manage a defined caseload and undertake comprehensive mental health assessments.
• Develop and evaluate individualised care plans and deliver evidence-based interventions for older adults.
• Collaborate with patients, carers, primary care services, health and social care professionals, and voluntary agencies.
• Participate in multidisciplinary meetings and case conferences, and contribute to service improvement initiatives.
• Provide leadership within the nursing team and support recruitment activity.
