Main duties of the job

You will be responsible for:

  • Holistic Assessments: Conduct thorough psychosocial and risk assessments in a range of settings including clients' homes, acute hospitals and community venues.
  • Care Planning: Develop and implement safe, creative and evidence-based care plans tailored to the individual's recovery needs.
  • Crisis Intervention: Deliver short-term therapeutic interventions such as emotional skills work, behavioural activation and psychoeducation.
  • Medication Management: Administer medication safely within the community and monitor for side effects.
  • Collaborative Working: Work closely with families, carers and professionals across health and social care to ensure continuity of care.
  • Supervision and Support: Provide leadership and supervision to Band 5 staff, students and support workers.
  • Service Development: Contribute to quality improvement initiatives and the ongoing development of urgent care services in Somerset.

You will need excellent interpersonal and clinical skills, a flexible approach to shift work, and a commitment to providing compassionate, person-centred care.
