Job summary

HCRG Care Group is seeking a Senior Community Health Navigator to join our team based at Kempthorne House. You will be the first point of contact for service users accessing BSW Community Health Services SPA, handling calls, emails and post with empathy and professionalism.

The role requires high administrative and IT skills, signposting and care navigation, while maintaining confidentiality. 37.5 hours per week, Derby Court, BA14 0XQ.

Qualifications

• Maths and English GCSE (A-C) or equivalent qualification level
• Administration Experience
• IT Systems Experience
• Proficient with Microsoft Office (Outlook, Word, Excel) and data entry
• Excellent interpersonal, organisational and communication skills
• Ability to maintain confidentiality and work with discretion
• Strong planning, time management and ability to meet deadlines
• Ability to work as part of a team

Responsibilities

• Manage information confidentially and liaise with professionals as required
• Act as first point of contact for all service users for BSW Community Health Services SPA
• Respond to calls, Direct and provide signposting / support
• Assess information and direct calls/referrals following correct processes
• Demonstrate empathy in difficult calls and de-escalate where needed
• Ensure accurate data input to IT systems and patient records
