Lancashire and South Cumbria NHS Foundation Trust is seeking a Band 6 Community Mental Health Practitioner for the Chorley area. You will join the Chorley CMHT and provide comprehensive assessments and care planning within specified pathways for highly complex cases.

You will work in a multidisciplinary team, contribute to risk management, and support supervision of colleagues. The Trust emphasises development, flexibility, and compassionate care across the community.

Qualifications

• Core registered mental health practitioner with post-qualification experience.
• Knowledge of psychotropic medications including Clozaril and Lithium.
• Experience in risk assessment and care management.
• Ability to work in a multidisciplinary team with complex needs.

Responsabilités

• Provide comprehensive assessment, advice and treatment to people with highly complex needs referred to Community Services in accordance with care pathways for given specialism.
• Deliver specialist interventions in accordance with care pathways for given specialism.
• Work closely with other professionals within and outside the Community Team.
• Actively supports the Team Leader/Advanced Practitioner in KPI/CQUIN targets and regulatory registration.
